A half million people are diagnosed every year with oral cancer. While other cancers are more prevalent, oral cancer is among the most dangerous with only a fifty percent five-year survival rate.
A major reason for this low rate is because this fast growing cancer is difficult to detect early — diagnosis comes far too often after the disease has already well advanced. In an effort to detect cancer earlier many dentists visually screen for oral abnormalities during checkups, especially patients over fifty, tobacco or heavy alcohol users, patients with a family history of cancer or a medical history of exposure to the sexually transmitted human papilloma virus, HPV-16.
If they detect an abnormality, the dentist often refers the patient to an oral surgeon or other specialist for a possible biopsy. In this procedure the surgeon removes a sample of the abnormal tissue, which is then examined microscopically for cancer cells. A biopsy remains the most effective way to diagnose oral cancer.
Because of the disease's aggressive nature, many dentists lean to the side of caution when referring patients for biopsy. As a result 90% of oral biopsies reveal no cancer. Reducing the number of biopsy referrals is highly desirable, especially for the patient undergoing the procedure. Tissue samples tend to be large to ensure complete detection of any cancer cells. Depending on the size and location of the sample, there may be a risk for loss of function or disfigurement.
A new screening tool using a sample of a patient's saliva could help reduce the number of biopsy referrals. Besides DNA, saliva also contains dormant genes called biomarkers that activate in response to the presence of a specific disease. This particular saliva test identifies those biomarkers for oral cancer if they're present.
A sample with a low score of biomarkers indicates no cancer present (with a statistical confidence of 99%). A medium or high score indicates cancer may be present, but only a biopsy can determine for sure. Using this test, dentists might be able to reduce the number of biopsy referrals and instead be able to employ watchful waiting in certain cases. Because of its simplicity and non-invasiveness, saliva screening could help identify oral cancer earlier.

Root canal therapy. Tooth decay can do more than cause cavities — it can work its way into the pulp, the innermost layer of a tooth. If it isn't stopped here, it could continue on to the roots and put the tooth in real danger of loss. A root canal treatment removes the infection from the pulp and root canals and replaces the space with a filling. A life-like crown is then bonded or cemented to the tooth to protect it from further infection.
Aggressive treatment for periodontal (gum) disease. This other dental disease is just as damaging as tooth decay. Caused by bacterial plaque, the gums around a tooth become infected and inflamed. As it moves deeper into the tissues and inflammation progresses, it can affect supporting bone causing it to dissolve. To prevent this potential bone loss, it's important to seek out and remove hidden pockets of plaque. This may require surgery to access the roots for plaque and calculus (tartar) removal, but it's well worth it to preserve the tooth.
Bone grafting. As mentioned before, gum disease can ultimately lead to bone loss. But even when bone loss has occurred (a substantial threat to a tooth's survival) we may be able to reverse it with bone grafting techniques. During this procedure we insert grafting material at the loss site along with substances that stimulate growth. The graft serves as a scaffold for new bone cells to grow upon. Over time the bone volume increases and helps stabilize a weak tooth.
Of course, your best option is to avoid dental disease in the first place with daily brushing and flossing and regular dental visits for cleanings and checkups. That and treating dental disease in its earliest stages will help ensure you'll have the best teeth possible — your own.

Losing permanent teeth is never good — unlike primary teeth, no natural replacements wait in the wings. But the good news is you have a number of options for replacing them with life-like prosthetic (false) teeth.
Today's premier choice is dental implants, preferred by dentists and patients alike for their durability and life-likeness. But because of their cost when replacing multiple teeth, many people opt for traditional dentures. And now dentures are easier to wear and maintain thanks to new, advanced materials and designs.
Still, there's one major area where implants have the definite edge over dentures — long-term bone health. Older bone cells die and dissolve (resorb), replaced then by newly formed cells. Teeth help perpetuate this cycle through the forces generated when we chew that travel through the roots to stimulate the formation of new bone.
But because this stimulation through a tooth ends when it's lost, new bone beneath the empty socket may not keep up with the resorption rate of older bone. As a result, you could lose as much as a quarter of normal bone width in just the first year after losing a tooth.
This bone loss will continue to accumulate even if you wear dentures, which can't replicate the bone growth stimulation of natural teeth. What's more, the constant pressure on the bony ridge of the gums can accelerate bone loss. Eventually, the firm, comfortable fit you first had with your dentures will become looser and less comfortable with the shrinking bone volume.
Implants, on the other hand, can stop bone loss and may even reverse it. This is because the titanium metal of an implant has a special affinity with bone cells that readily grow and adhere to it. This creates the anchorage responsible for the implant's durability, but it's also healthy for the bone.
Of course, this doesn't have to be a binary choice between the two restorations thanks to a new hybrid advancement that combines implants with dentures. We can install as few as two implants to support a removable denture. You'll enjoy greater stability, fit and durability with your dentures, while also improving bone health through the implants.

Our first step is to determine the best positioning for the prosthetic teeth. It's not an “eyeball” guess — we make a number of calculations based on the shape and size of your jaws and facial features to determine the best settings within the resin base. These calculations help us answer a few important questions for determining design: how large should the teeth be? How far forward or back from the lip? How much space between the upper and lower teeth when the jaws are at rest?
We also can't forget about the artificial gums created by the base. How much your gums show when you smile depends a lot on how much your upper lip rises. We must adjust the base size to accommodate your upper lip rise so that the most attractive amount of gum shows when you smile. We also want to match as close as possible the color and texture of your natural gum tissues.
There's one other important aspect to manage: how your upper and lower dentures function together when you eat or speak. This means we must also factor your bite into the overall denture design. This may even continue after your dentures arrive: we may still need to adjust them while in your mouth to improve function and comfort.

There's a lot of emphasis — well-placed, of course — on preventing and treating tooth decay. But there's another dental disease just as dangerous to your oral health and nearly half of U.S. adults have it. It's actually a group of diseases known collectively as periodontal (gum) disease.
Gum disease is similar to tooth decay in one respect: they're both triggered by bacteria. These microorganisms thrive in a thin film of food particles called plaque that collects on tooth surfaces.
Certain bacteria can infect gum tissues and trigger inflammation, a response from the body's immune system to fight it. As the battle rages, bone loss can occur and the gums weaken and begin to detach from the teeth. Without treatment, you could eventually lose affected teeth.
Like tooth decay, the best approach with gum disease is to prevent it, and by using the same techniques of daily brushing and flossing. These actions loosen and remove plaque built up since your last brushing. It's also important you visit us at least twice a year for cleanings that remove hard to reach plaque and calculus (hardened plaque deposits).
If despite your best efforts you do contract gum disease, the sooner you see us for treatment the lower the long-term impact on your health. The treatment aim is the same as your daily hygiene: to remove plaque and calculus. We use specialized hand instruments or ultrasound equipment to mechanically remove plaque; more advanced cases may require the skills of a periodontist who specializes in caring for structures like the gums that support teeth.
So, defend yourself against gum disease by brushing and flossing daily, and visiting us regularly for dental cleanings and checkups.
