There are a lot of reasons (including a blow to the mouth) why one of your permanent teeth might become loose. The most common: advanced periodontal (gum) disease that has weakened the gum attachment to the tooth.
There's also another, less common reason: you have a grinding habit that's producing higher than normal biting forces. Besides accelerating tooth wear, the constant jaw movement and teeth clenching can stretch periodontal ligaments and loosen their attachment to a tooth.
If the gums are disease-free, teeth grinding is most likely the main culprit for the damage, what we call primary occlusal trauma. Our treatment goal here is to reduce the effect of the grinding habit and, if necessary, secure the teeth with splinting while the ligaments heal. We can often reduce the grinding effect with a custom bite guard worn while you sleep. We may also prescribe minor muscle relaxants and mild pain medication like aspirin or ibuprofen.
Sometimes we may need to perform other measures like re-shaping your teeth's biting surfaces so they don't generate as much biting force. You may also benefit from counseling or other psychological treatment to help you address and cope with stress, a prime driver for teeth grinding.
Even if you don't have a grinding habit, biting forces may still contribute to tooth looseness if you have advanced gum disease. Advanced disease results in excessive bone loss, which in turn reduces the remaining amount of ligaments attached to the tooth. This type of damage, known as secondary occlusal trauma, and ensuing tooth looseness can occur even when your biting forces are normal.
It's necessary in these cases to treat the gum disease, primarily by manually removing plaque and calculus (hardened plaque deposits), which causes and sustains the infection. Once removed, the gums can begin to heal and strengthen their attachment. We may also need to apply splinting or perform surgical procedures to encourage gum and bone reattachment.
Whatever has caused your loose tooth, our goal is to remove the cause or lessen its effects. With your tooth secure and the gums regaining their healthy attachment, we have a good chance of saving it.

Teeth grinding is one childhood habit that sounds worse than it usually is: often the most harm done is to your night’s sleep. That said, though, it’s still a habit to keep your eye on.
Also known as bruxism, teeth grinding is so common among children that it’s considered normal behavior by many healthcare professionals. As for causes, some suggest a child’s immature neuromuscular chewing control may trigger it, while others point to the change from deeper sleep to a lighter stage as a possible cause. Problems like airway obstruction, medications or stress also seem to contribute to the habit.
For most children, teeth grinding usually fades by age 11 with no adverse effect on their teeth. If the habit extends into adolescence, however, there’s an increased risk for damage, mainly tooth wear.
This can happen because grinding often produces chewing forces 20-30 times greater than normal. Over time this can cause the biting surfaces of the teeth to wear and reduces the size of the teeth. While teeth normally wear over a lifetime, accelerated wear can pose a significant health risk to your teeth. Any sign of tooth wear in a child or adolescent is definitely cause for concern.
If your child’s tooth grinding habit appears to be developing into a problem, your dentist may recommend a few treatment options. The most common is a thin, plastic night guard worn in the mouth during sleep that prevents the upper and lower teeth from making contact. If the suspected cause is airway obstruction, they may refer you to an ear, nose and throat (ENT) specialist to seek treatment for that, as well as other professionals to help with managing stress or medications.
Like thumb sucking, the habit of teeth grinding usually ends with no permanent ill effects. But if you notice it continuing late into childhood or your dentist finds tooth wear or other problems, take action to avoid problems long-term.

Your teeth naturally wear as you age, but you may be making it worse if you grind your teeth.
Teeth grinding is a behavior that causes the teeth to gnash, grind or clench against each other generating forces greater than those produced from normal biting. These forces often result in tooth wear that cause not only functional problems but result in a more aged appearance. Grinding occurs while a person is awake, but most often episodes occur while asleep at night.
Teeth grinding is quite common in children, but not usually of great concern since most grow out of it. There's even a school of thought that teeth grinding might even help readjust an uneven bite.
Among adults, though, other factors seem to contribute to teeth grinding. Many researchers believe nighttime grinding occurs as a person passes through different sleep phases including deep REM (Rapid Eye Movement) sleep. It may also have a connection with chronic snoring.
Certain medications seem to contribute to teeth grinding, particularly psychoactive drugs like amphetamines. Nicotine falls in this category, which could be why tobacco users report twice the incidence of the habit compared to non-users. Teeth grinding is also connected to another fact of modern life: stress. People who grind their teeth tend to have higher levels of anxiety, hostility or depression.
Because there are multiple triggers, there are many treatment approaches. Whatever course we take, our aim is to eliminate or minimize those factors that contribute to your habit. For example, we can create a custom mouth guard for night wear to prevent the teeth from making solid contact and thus reduce the biting pressure.
Perhaps the most important thing is to control or reduce stress. This is particularly helpful at night to prepare you for restful sleep by changing some of your behaviors. We also encourage investigating other stress therapies like biofeedback, meditation or group therapy.
Whatever the means, bringing teeth grinding under control not only reduces problems now, but could also help prevent abnormal teeth wearing and future health issues down the road.

Clenching, or grinding of your teeth (also known as bruxing) are common habits. Biting forces are normally small, gentle, fleeting and very frequent throughout the day. In fact, it's the normal stimulus necessary to keep your teeth and jawbone healthy. When you clench or grind your teeth you apply forces up to ten times normal (in the 200 lb range). And it's not just the force, it's the duration and frequency with which they're applied. High forces lasting for seconds or minutes, frequently exerted, can affect some or all of the masticatory system. This includes the teeth, jaws, jaw joints and muscles, causing aching jaws, headaches, earaches, neck and even backaches; and the teeth themselves causing excessive wear, fractures, or even loose teeth.
Why does grinding occur? Habitual grinding is most frequently a reaction to stress. Sometimes abnormalities in your bite or malocclusion (“mal” – bad; “occlusion” – bite) can trigger clenching or grinding. It is normal for children to sometimes grind their teeth when new teeth are coming in, but it may be indications of stress too. You may be grinding your teeth subconsciously in your sleep, but it may be so loud as to awaken your sleeping partner. Or our office may be the first to suspect it during a dental exam because of the apparent signs of change to muscles, joints and teeth especially abnormal tooth wear.
What can be done about teeth grinding or bruxing? If you are symptomatic, having pain, muscle, joint or tooth soreness, the first step is to get you comfortable. Generally, a mild non-steroidal anti-inflammatory medication (aspirin, ibuprofen) and muscle relaxants will help in addition to moist heat and mild jaw exercises. Stress management is also helpful. To prevent further damage, we may recommend a bite guard made of wear-resistant plastic that fits over the biting surfaces of your upper teeth. These customized unobtrusive appliances when properly fitted and adjusted stop clenching and grinding activity, or at least the damage they can do. A bite guard can be worn day or night especially during stressful periods.
